Legacy Health is a regional six-hospital health system offering a full range of primary and tertiary care services for both adults and children at sites throughout the Portland metropolitan and southwest Washington areas. We provide an integrated network of healthcare services, including acute and critical care, inpatient and outpatient treatment, community health education and a variety of specialties. We have an exciting opportunity for a system wide Director for Care Management. This position will provide strategic leadership and operational oversight for our care management program across all settings of care, including RN case management, discharge planning, utilization management; and clinical documentation improvement programs. Reporting to the System Chief Nursing Officer, this position will also serve a central role in the development of our care transformation work across Legacy.

In addition to exceptional leadership and communication skills, the ideal candidate will have appropriate graduate education, a strong background in clinical nursing, and a demonstrated expertise in case management, social work, utilization management. Oregon RN licensure or eligibility.
